Thank you for the opportunity to endorse your product.  It is a wonderful convenience for both our team and our patients.  It saves us literally hours each week.  Since almost all of our patients are new, we have to fill out many forms and enter the information in the computer.  Prior to using your service, the patient had several minutes to wait upon arrival before they could be seen.  It now takes only seconds.  We know who they are before they walk through the door.

How It Works

How our HIPAA compliant form service works

  • We create the online form on our 256-bit encrypted secure server, based on your in-office form. There will be no "Secure Forms Vault" page header or menu at the top. The page will look very generic, with your logo, address, phone number, and web address at the top. See our "DEMO Practice" form.
  • You would then put a link on your website to your form that resides on our  secure server
  • Your patient goes to your website, clicks the link and completes the form
  • When your patient submits their completed form:
    • your patient gets a "form submitted successfully" message and is automatically returned to your website in 5 seconds.
    • Your office is notified via email that a new patient form has been submitted
    • Your patient is also emailed a "Thank You" for completing their forms online that looks like it is from your office
  • You simply log into your SecureFormsVault.com account to see a list of completed forms for your office, with the newest submitted forms at the top
  • Forms can then be printed, exported to PDF, Exported to CSV (Microsoft Excel), and deleted when you are done if you want.
